- Eye Exam – $10 copay for eye exams
- Frames – $130 allowance for frames with a 20% discount off balances over $130
- Lenses – $25 copay for standard, $90 copay for progressive, plus additional coverage for lens options
- Contacts – $130 allowance with 15% off balances over $130
